Consultation Agreement for Ask Dr Olz (Pty) Ltd

Welcome to Ask Dr Olz (Pty) Ltd.
By booking a telehealth consultation through our platform, you agree to the terms outlined in this agreement. Please read the following carefully before proceeding with your booking.

1. Nature of Telehealth Services
You understand that the consultation will be conducted via telehealth, which involves remote healthcare services through electronic communication. This includes video, audio, or text-based consultations. There will be no in-person physical examination.
– Telehealth consultations will be based on the medical information you provide.
– Your practitioner will advise on treatment and wellness plans based on your disclosed health concerns and history.

2. Confidentiality and Data Protection
We take your privacy seriously. All information shared during your consultation is confidential and protected in compliance with South African privacy laws.

– We use secure, encrypted platforms to ensure the confidentiality of all patient data.
– Despite security measures, there may be inherent risks, such as data breaches, associated with electronic communication.
By agreeing to this consultation, you acknowledge these risks.

3. Payment Terms
By proceeding with your booking, you agree to the following fees, depending on the consultation package selected:
– Premium Package: R950 (45-minute consultation, including a free follow-up worth R500 within 45 days).
– Standard Package: R600 (30-minute consultation, including a follow-up worth R300 within 45 days).
– Basic Package: R250 (15-minute consultation, no follow-up included).
Additional follow-ups may be booked separately:30-minute follow-up consultation: R500 – 15-minute follow-up consultation: R300 Payment will be due at the time of booking, and all prices include VAT.

4. Cancellation and Refund Policy
You may cancel or reschedule your consultation up to 24 hours before the appointment without incurring charges. Late cancellations or no-shows will be
subject to the full consultation fee. Refunds for unused services or packages will be processed in accordance with our Refund Policy, available on our website. 

5. Patient Responsibilities
You agree to:
– Provide complete and accurate medical information prior to the consultation.
– Follow the advice provided by your healthcare practitioner and update them on any changes in your condition.

6. Limitation of Telehealth Services
You understand that telehealth services have limitations, such as the inability to conduct physical examinations. Diagnosis and treatment are based solely on the
information you provide.
– In some cases, you may be referred to another healthcare provider or recommended for in-person consultation or diagnostic tests.

7. Acceptance of Terms
By clicking “Agree,” you acknowledge that you have read and understood the terms outlined in this Consultation Agreement. You consent to receiving telehealth services from **Ask Dr Olz (Pty) Ltd** and agree to the associated costs, cancellation policies, and limitations of telehealth services.
