1. Purpose

This agreement outlines the terms under which you (“the Patient”) engage Ask Dr Olz (Pty) Ltd (“the Practice”) for telehealth and/or in-person consultations. By booking, you provide informed consent to receive healthcare services under these terms.

2. Nature of Services

  • Services are provided by a registered Homeopath under the Allied Health Professions Council of South Africa (AHPCSA).

  • Consultations may include health assessments, dietary/lifestyle recommendations, supplement recommendations, and natural treatment protocols.

  • Telehealth consultations are conducted via a HIPAA-compliant platform to ensure confidentiality.

  • The advice provided is not a substitute for emergency medical care.

3. Patient Responsibilities

You agree to:

  • Provide full, truthful, and accurate health information.

  • Inform the practitioner of all current medications, supplements, and existing medical conditions.

  • Follow agreed recommendations to the best of your ability.

  • Contact emergency services in the event of a medical emergency.

4. Telehealth Consent

By booking a telehealth consultation, you acknowledge:

  • You understand the benefits and limitations of remote healthcare.

  • You consent to the use of secure video conferencing, phone, and/or electronic communication.

  • Technical issues may occasionally disrupt consultations.

5. Fees and Payment

  • All consultations are private pay; no direct medical aid billing is offered.

  • You may submit receipts to your medical aid for reimbursement (if applicable).

  • Payment is required at the time of booking via PayFast or Peach Payments.

  • Consultation Duration: Initial – 60 mins; Follow-up – 30 mins.

6. Cancellations & Rescheduling

  • You may cancel or reschedule up to 24 hours before your appointment without penalty.

  • Late cancellations or missed appointments will be charged the full consultation fee.

  • Refunds are processed according to our Return & Refund Policy.

7. Confidentiality & Records

  • Your personal and health information is kept confidential in line with POPIA and relevant privacy laws.

  • Records are stored securely and only accessed by authorised personnel.

8. Consent to Treatment

By booking, you confirm that:

  • You understand the nature, purpose, and potential outcomes of the consultation.

  • You understand that no guarantees can be made regarding specific health outcomes.

  • You voluntarily consent to receive care from Ask Dr Olz (Pty) Ltd.

9. Acknowledgement

By proceeding with your booking and payment, you acknowledge that you have read, understood, and agree to the terms in this Consultation Agreement & Consent.
